An accident may occur due to single or multiple cause(s) and lack of knowledge and safety awareness among management, workers, visitors and public alike. Since the implementation of Occupational Safety and Health Act (OSHA) in 1994, the safety awareness has increased tremendously at the work place. Safety and health procedures implementation is the most important factor in preventing accidents. Among the responsibilities of the management towards safety is to provide a safe working environment and provision of personal protective equipment to all workers and visitors. The other responsibility of the management is to provide continuous safety awareness training to all its workers.
